NOTICE: Influenza Vaccination
Hongo Health Service Center will give the influenza vaccination on the following schedule.
- Date: Friday, 8 November - Thursday, 14 Novermber (except Saturday and Sunday)
- Venue: 1F, Multipurpose room, Hongo Health Service Center
- Vaccination fee for students: 2,000JPY
* Please have exact change ready.
* Online pre-registration is required for the vaccination. The registration page will be available from Wednesday, 23 October.
When you get a flu vaccination, please print the "Influenza Vaccination Questionnaire" on the both sides of a paper, complete it
in advance and bring it with your student ID card and the registration form to the Health Service Center.

【事務連絡】インフルエンザの予防接種について
保健・健康推進本部では、下記の日程で予防接種を実施します。
- 日程: 11月8日（金）～ 11月14日（木）（土日を除く）
- 場所：本郷保健センター１階 多目的室
- 接種料金（学生）：2,000円（釣銭のないようにお願いします。）
* 接種希望者は保健・健康推進本部Webサイトでの事前予約が必要です。予約は10月23日（水）から可能です。
接種当日は学生証及び予診票・予約票を必ず持参してください。予診票はwebサイトからダウンロードし両面印刷後、必要事項を記入願います。
