NOTICE: Influenza Vaccination
Hongo Health Service Center will give the influenza vaccination on the following schedule.
- Date&Time: Friday, 6 December from 1pm
- Venue: 2F, Otolaryngology (ENT) department, Hongo Health Service Center
- Vaccination fee for students: 2,000JPY
* Please have exact change ready.
* Online pre-registration is required for the vaccination. The registration page will be available from Friday, 29 November to Friday, 6 December 9pm.
When you get a flu vaccination, please print the "Influenza Vaccination Questionnaire" on the both sides of a paper, complete it in advance and bring it with your student ID card, the registration form and the fee to the Health Service Center.
【事務連絡】インフルエンザの予防接種（追加）について
保健・健康推進本部では、下記の日程で追加の予防接種を実施します。
- 日程: 12月6日（金）午後
- 場所：本郷保健センター2階 耳鼻咽喉科室
- 接種料金（学生）：2,000円（釣銭のないようにお願いします。）
* 接種希望者は保健・健康推進本部Webサイトでの事前予約が必要です。予約は11月29日（金）から12月6日（金）午前9時までに行ってください。
接種当日は学生証、インフルエンザ予診票、予約票と接種料金を必ず持参してください。予診票はwebサイトからダウンロードし両面印刷後、必要事項を記入願います。
