NOTICE: Influenza Vaccination
Hongo Health Service Center will give the influenza vaccination on the following schedule.
- Date: November 4, 5, 6, 10, 13, and 14
- Venue: Hongo Health Service Center, 2F of Administration Bureau Bldg.2
- Vaccination fee for students: 2,000JPY
* Please have exact change ready.
* Online pre-registration is required for the vaccination. The registration page will be available from Friday, October 24, 2014.
When you get a flu vaccination, please print the "Influenza Vaccination Questionnaire" on the both sides of a paper, complete it
in advance and bring it with your student ID card and the registration form to the Health Service Center.
You can download the "Influenza Vaccination Questionnaire" from the Health Service Center's website.
Also, please bring registration information wiht a print of screen, a memorandum, or a screen image.
【事務連絡】インフルエンザの予防接種について
保健・健康推進本部では、下記の日程で予防接種を実施します。
- 日程: 11月4日（火）、5日（水）、6日（木）、10日（月）、13日（木）、14日（金）
- 場所：第2本部棟2階 本郷保健センター
- 接種料金（学生）：2,000円（釣銭のないようにお願いします。）
* 接種希望者は保健・健康推進本部Webサイトでの事前予約が必要です。予約は10月24日（金）から可能です。
接種当日は学生証及び予診票・予約情報（画面印刷・メモ・画像可）を必ず持参してください。予診票はwebサイトからダウンロードし両面印刷後、必要事項を記入願います。
