Notice: Influenza Vaccination
Hongo Health Service Center will give the influenza vaccination on the following schedule.
- Date and Time:
10:00-12:30 Monday, 5 November 2012
13:30-16:00 Tuesday, 20 November 2012
*Each day the first 400 people can be vaccinated. - Venue: 1F, Multipurpose room, Hongo Health Service Center
- Vaccination fee: 2,000JPY *Please have exact fare.
If you would like to get a flu vaccination, please fill in the "Influenza Vaccination Questionnaire" in advance and bring it with your student ID card to the Health Service Center.

事務連絡：インフルエンザの予防接種について
保健・健康推進本部では、下記の日程で予防接種を実施します。
- 日時：
2012年11月5日（月）10:00-12:30
2012年11月20日（火）13:30-16:00
（各日先着４００名までとなります。） - 場所：本郷保健センター１階 多目的室
- 接種料金：2,000円（釣銭のないようにお願いします。）
当日は、あらかじめ記入したインフルエンザ予診票および学生証を持参してください。
